There are no hereditary ranks; all have to be earned. Once the rank of Commander is attained, there are two options - one is the military focused Marshal ranks (Marshal -> Quartermaster Marshal -> Sergeant Marshal -> Lord Marshal) or the politically orientated Lord titles (Lord -> High Lord -> Lord Lieutenant -> Lord Commander). Grand Marshals are appointed by the Supreme Lord to the Eight Point Table from among the Marshal Lords and Lord Commanders. The Supreme Lord is elected from among themselves by the Grand Marshals.

A Quartermaster Marshal outranks, and can give order to, a Lord, similarly for a High Lord viz. a Marshal. Once a path is chosen, you cannot switch between them.
